Description
Temporal operculum cortex forms the inferior lip of the lateral sulcus (Sylvian fissure) along the superior surface of the temporal lobe. From a lateral cerebral hemisphere perspective, the opercular rim sits inferior to the frontal and parietal opercula and partially overlies the insular cortex deep within the fissure, with the superior temporal gyrus defining much of its external contour. Posteriorly, the opercular margin approaches the region of the planum temporale, while anteriorly it blends toward the temporal pole. Short association cortex dominates the exposed surface. For neuroanatomy teaching, the temporal operculum is one of the best fixed landmarks for orienting the lateral fissure and the hidden insula, and it clarifies why the insular gyri are not visible on routine external inspection of the cerebrum. In the operating room, sylvian fissure splitting for middle cerebral artery aneurysm clipping and for insular or peri-insular tumor access depends on a precise sense of the superior temporal gyrus and its opercular edge, because the MCA trunk and its M2 branches course in the depth of this fissure adjacent to opercular pia. Localization matters. The same surface relationships also support discussions of epileptogenic networks near the perisylvian cortex and of language-auditory anatomy, including proximity to primary auditory cortex on Heschl’s transverse temporal gyri, which lie on the superior temporal plane just deep to the operculum.
