Description
Centered within the anterior superior mediastinum, the thymus sits immediately posterior to the manubrium and upper sternal body and anterior to the pericardium and great vessels. In a standing male child, the gland typically extends superiorly toward the root of the neck, approaching the thyroid region, while its inferior margin tapers toward the level of the fourth costal cartilages. Laterally, its lobes abut the mediastinal pleura and the medial surfaces of the lungs, with the brachiocephalic veins and superior vena cava lying posterior and slightly rightward. This is a midline landmark. Pediatric thymic size and contour are common sources of confusion on chest radiography and CT, where a normal thymus can project as an anterior mediastinal “sail sign” or appear prominent after stress recovery. Clear localization in relation to the sternum and pericardium helps distinguish normal thymic tissue from anterior mediastinal masses such as lymphoma, germ cell tumors, or thymic enlargement in Graves disease. For procedural planning, knowing the thymus’ proximity to the left brachiocephalic vein and internal thoracic vessels matters during mediastinal surgery and when interpreting complications after central venous access. Use this rendering in pediatric anatomy teaching for the thorax and lymphoid organs, in radiology education when correlating surface anatomy to mediastinal compartments, and in patient-facing materials explaining why a child’s thymus can look large on imaging.
