Description
Seen from a posterior perspective in an adult male, the paired ureters descend retroperitoneally from the renal pelvis at the medial hila of the kidneys and track inferiorly toward the urinary bladder. Each ureter runs anterior to psoas major and is crossed anteriorly by gonadal vessels as it approaches the pelvic brim, while remaining lateral to the abdominal aorta on the left and the inferior vena cava on the right. Superiorly, the lower lobes of the lungs frame the upper abdomen, with the liver occupying the right upper quadrant and the spleen on the left, and the small and large intestines lying more anteriorly than the ureteral course. Relationships are the point. This posterior layout matters because it mirrors the retroperitoneal anatomy encountered on CT urograms and during posterior or flank exposure to the kidney, where ureteral identification prevents iatrogenic injury. Narrowings at the ureteropelvic junction, at the crossing of the iliac vessels, and at the ureterovesical junction are classic sites of ureterolithiasis, and a posterior view helps correlate referred pain patterns from the costovertebral angle to the groin. Clinicians also use these landmarks when planning ureteral stenting, ureteroscopy, or managing obstruction from retroperitoneal fibrosis or an expanding abdominal aortic aneurysm.
