Posterior abdominal anatomy centers on the left and right kidneys with their renal pelves narrowing into the ureters, paired muscular tubes that descend inferiorly on the anterior surface of the psoas major toward the true pelvis. Medial to each kidney run the abdominal aorta (left) and inferior vena cava (right), with the renal arteries and renal veins crossing laterally into the renal hila, and paraaortic and paracaval lymph nodes tracking alongside the great vessels. The ascending and descending colon frame the retroperitoneum laterally, while autonomic nerve fibers (lumbar splanchnics and the aortic plexus) course near the ureter. A long retroperitoneal run. For retroperitoneal surgery, the ureter is a practical landmark on the psoas, and its proximity to gonadal vessels and colonic mesentery explains why iatrogenic ureteral injury remains a concern in colectomy, aortoiliac reconstruction, and lymph node dissection. Hydronephrosis, ureteral stricture, and retroperitoneal fibrosis make these planes clinically familiar.
