Description
Running retroperitoneally from the renal pelvis to the urinary bladder, the male ureter appears as a slender muscular tube descending on the anterior surface of the psoas major and crossing the pelvic brim near the bifurcation of the common iliac vessels. From an anterior perspective, the ureter sits posterior to the parietal peritoneum and medial to the ascending and descending colon, with jejunoileal loops draped more anteriorly where the mesentery allows. Superiorly it relates to the renal hilum and renal vessels, while inferiorly it tracks toward the posterolateral bladder base, approaching the ureterovesical junction. Major landmarks. The abdominal aorta and inferior vena cava run vertically in the midline, with segmental branches and accompanying veins framing the ureter’s course. This angle matters because it mirrors how the ureter is found and protected during open and minimally invasive abdominal and pelvic surgery, where the main hazard is inadvertent ligation, thermal injury, or transection during mobilization of the colon or dissection near the iliac vessels. It also supports teaching of ureteral narrowing sites that trap calculi, at the ureteropelvic junction, at the pelvic brim where it crosses the iliac vessels, and at the intramural segment entering the bladder, all common pain generators with referred discomfort to the groin and testis via shared segmental innervation. Use it for gross anatomy and urology teaching when you need a clean, anterior roadmap of retroperitoneal relationships, or for operative anatomy figures in colorectal, vascular, and pelvic surgery texts discussing ureter identification during left and right colon mobilization.
