Description
Anterior pelvic anatomy centers on the uterus, with the uterine arteries approaching the cervix at the level of the internal os and then ascending in a tortuous course along the lateral uterine wall within the broad ligament. Arcuate arteries run circumferentially in the myometrium, giving rise to radial arteries that extend toward the endometrium and continue as straight arteries to the stratum basalis and spiral arteries to the stratum functionalis. Venous drainage parallels the arterial pattern, forming a uterine venous plexus along the uterine margins and communicating with the vaginal plexus inferiorly. Ovarian artery anastomoses along the uterine cornu are typically indicated where the tubal and ovarian branches meet the uterine supply. These circulatory pathways matter because they explain both normal cyclic endometrial change and the bleeding patterns seen in common gynecologic disease. Spiral artery remodeling under progesterone influence, and its breakdown with withdrawal, underlies menstrual shedding; disordered remodeling is part of abnormal uterine bleeding. For operative anatomy, the relationship between the uterine artery and the ureter (water under the bridge) guides uterine artery ligation during hysterectomy and helps avoid ureteric injury, while the rich myometrial arterial arcade explains why postpartum hemorrhage responds to uterotonic agents and, when needed, uterine artery embolization.
