Description
Seen from an inferior (caudal) perspective, the inferior vena cava ascends in the retroperitoneum and approaches the central tendon of the diaphragm to enter the thorax through the caval hiatus. The vessel sits to the patient’s right of midline, with the abdominal aorta positioned more posterior and slightly left, and the right atrium lying superior to the diaphragmatic opening. Hepatic veins converge on the cava just inferior to the diaphragm, while the surrounding diaphragmatic fibers frame the hiatus within the tendinous portion rather than the muscular crura. Orientation is unmistakable. That relationship matters because the caval hiatus is not just a hole, it is an anatomic corridor that changes with respiration and with intra-abdominal pressure, influencing venous return to the heart. This view offers a clean way to teach why Budd-Chiari syndrome (hepatic venous outflow obstruction) produces upstream hepatic congestion near the hepatocaval junction, and why tumor thrombus from right renal cell carcinoma can extend proximally into the inferior vena cava and toward the right atrium. It also supports procedural planning for caval filter placement and for hepatic surgery where short hepatic veins and the suprahepatic cava are high-risk sources of bleeding.
