A malignant tumor mass emerges within one adrenal, expanding the normally small, triangular gland into a lobulated neoplasm that displaces nearby fat planes and subtly alters the silhouette of the upper renal pole. As the sequence progresses, bone and soft-tissue opacity are balanced to keep ribs, thoracolumbar vertebrae, and the upper abdomen as fixed landmarks while the cancerous focus is brought forward in depth.
