Description
Beginning with a normal frontal chest radiograph, the animation centers on the thymus in the anterior mediastinum, just posterior to the manubrium and anterior to the pericardial silhouette, with the upper lobes of the lungs framing it laterally. As the sequence progresses, a thymic neoplasm enlarges superiorly toward the thoracic inlet and inferiorly toward the cardiac border, distorting the normally smooth anterior mediastinal contour and breaking left to right symmetry. Mediastinal lines and interfaces shift in real time, including the retrosternal clear space and adjacent lung margins, so the viewer can track mass effect as it develops. Thymic carcinoma and other malignant thymic tumors often present as an anterior mediastinal mass on chest x-ray before cross-sectional imaging is obtained, and subtle asymmetry can be the earliest clue. Motion adds teaching value here because the animation can step through staged radiographic changes, showing how a growing lesion widens the mediastinum, silhouettes adjacent borders, and may suggest invasion or compression patterns that raise concern for superior vena cava obstruction. Differential diagnosis of an anterior mediastinal mass is not academic: distinguishing thymic malignancy from lymphoma, substernal thyroid goiter, or germ cell tumor directs the next study and the biopsy approach.
