Description
Radiographic anatomy of the anterior mediastinum comes into focus as the thymus sits posterior to the sternum and anterior to the pericardium, tapering superiorly toward the lower neck. Over the sequence, a thymic neoplasm expands within the thymic silhouette, forming an irregular tumor mass that projects over the cardiac shadow and lies medial to the adjacent lung fields. As the animation progresses, the mass effect is read in relation to nearby landmarks, including the tracheal air column, the aortic arch contour, and the expected course of the superior vena cava. Thymic carcinoma is an uncommon but aggressive anterior mediastinal malignancy, and on plain-film type visualization it can be confused with lymphoma, retrosternal thyroid disease, or a germ cell tumor when the borders are indistinct. The stepwise development of a radiographic density makes the teaching point clear: subtle widening of the mediastinum and loss of normal retrosternal lucency often precede an obvious bulky mass, and progressive enlargement helps explain symptoms from compression, including cough, dyspnea, or superior vena cava syndrome. Association with paraneoplastic autoimmunity, including myasthenia gravis, is a practical reminder that thymic pathology is often discovered during workup that begins outside oncology.
