Description
Paired ureters descend retroperitoneally from the renal pelvis at the medial hilum of each kidney, running inferiorly on the anterior surface of psoas major while remaining posterior to the peritoneum of the posterior abdominal wall. As the sequence progresses, the luminal diameter and folded mucosa are emphasized, and the smooth muscular wall is shown producing peristaltic waves that drive urine distally. Crossing the pelvic brim near the bifurcation of the common iliac vessels, each ureter enters the lesser pelvis and courses anteromedially to reach the posterolateral urinary bladder, where it traverses the bladder wall obliquely to open at the ureteric orifice of the trigone. Peristalsis matters here because it explains why urine flow persists independent of gravity and why pain from ureteric obstruction can radiate from the flank toward the groin as distension and spasm propagate along the duct.
