Description

Arising from the renal pelvis at the ureteropelvic junction, each ureter descends retroperitoneally from the kidney to the urinary bladder as a muscular tube with a narrow, slit-like lumen. The animation traces the course anterior to the psoas major, with the ureter remaining medial to the colon and passing from the posterior abdominal wall into the pelvis as it crosses the pelvic brim near the bifurcation of the common iliac artery. Three radiographic constrictions are emphasized in sequence: the ureteropelvic junction, the crossing over the iliac vessels, and the intramural segment at the ureterovesical junction, where the ureter runs obliquely through the detrusor to open at the trigone. Peristaltic waves propagate distally. No guesswork. Stone disease and instrumentation hinge on these landmarks. Calculi commonly lodge at the UPJ, at the iliac crossing, or at the UVJ, and the narrowing points highlighted here map directly to the classic pain migration pattern from flank to groin and to the sites of obstruction on CT KUB or intravenous urography with upstream hydronephrosis. In the male pelvis, the distal ureter’s relationship to the ductus deferens near the posterolateral bladder and its proximity to the seminal vesicles and prostate explain why pelvic surgery and endoscopic manipulation can threaten the ureter, and why stent placement relies on recognizing the intramural tunnel and ureteral orifice.
