Description

Progressive morphologic change in the female pancreas is traced from normal glandular contour to a space occupying malignant mass, most consistent with pancreatic ductal adenocarcinoma. The animation centers on the head, neck, body, and tail of the pancreas, with the lesion expanding within the parenchyma and deforming the main pancreatic duct (duct of Wirsung) as it courses toward the major duodenal papilla. As the tumor enlarges, it encroaches on adjacent landmarks, the C loop of the duodenum anterolaterally, the common bile duct posterolaterally in the pancreatic head, and the splenic vessels along the superior border toward the tail. Growth is relentless. Clinical meaning follows the anatomy. A mass in the pancreatic head can narrow the distal common bile duct and ampulla, correlating with painless obstructive jaundice, dark urine, and pale stools, while perineural spread and posterior extension toward the superior mesenteric plexus helps explain deep epigastric pain radiating to the back. The sequential build up of tumor burden clarifies why cross sectional imaging focuses on ductal caliber change, upstream pancreatic atrophy, and loss of fat planes with the superior mesenteric artery and vein, findings that separate resectable disease from borderline or locally advanced presentations. Seeing the ductal distortion and regional compression unfold over time also supports teaching the rationale for CA 19-9 interpretation and EUS guided biopsy targets in suspected carcinoma.
