Description
Beating in a steady physiologic rhythm, the adult male human heart is rendered with clear external landmarks, including the apex pointing inferolaterally to the left and the broad base oriented superiorly and posteriorly. The right ventricle occupies most of the anterior surface while the left ventricle forms the left border and apex; the right and left auricles (atrial appendages) sit superiorly, flanking the roots of the great arteries. During each cardiac cycle the myocardium visibly thickens and relaxes, with subtle torsion at the apex and synchronized expansion of the ascending aorta and pulmonary trunk as they arise superiorly from the ventricular outflow tracts. Coronary arteries and companion cardiac veins track over the epicardial surface, and the superior and inferior vena cava align with the right atrium on the right side of the mediastinum. Normal mechanics matter. Seeing coordinated atrial filling followed by ventricular systole makes it easier to relate surface motion to common bedside findings such as the apical impulse, physiologic splitting of S2, and the hemodynamic consequences when contraction becomes dyssynchronous in left bundle branch block or dilated cardiomyopathy.
