Description

Beginning with an anterosuperior view, the animation orients the male liver beneath the right hemidiaphragm, with the larger right lobe lateral and the smaller left lobe extending across the midline toward the left hypochondrium. Rotation and gentle zooming define the falciform ligament as it ascends to the anterior abdominal wall, separating the left lobe from the medial aspect of the right lobe along the anterior surface. As the organ turns inferiorly, the visceral surface comes into view with the fissure for the ligamentum teres and fissure for the ligamentum venosum framing the left sagittal fissure, while the gallbladder fossa and groove for the inferior vena cava mark the right lobe posteriorly. Segmental boundaries are then introduced in sequence, tracking Couinaud segments around the portal hilum. Orientation matters in the operating room. Hepatic lobes describe surface anatomy, but Couinaud segments define functional territories based on portal vein inflow and hepatic vein outflow, and that distinction drives planning for segmentectomy, living-donor transplantation, and trauma packing.
