Description

Sweeping close to the endocardial surface of the atria, the camera tracks along the pectinate muscles (musculi pectinati) as parallel ridges that project into the atrial lumen, most prominent in the right atrial appendage (auricula dextra) and extending anteriorly from the crista terminalis. Medial to these ridges, the superior and inferior venae cavae and the ostium of the coronary sinus relate to the smoother sinus venarum, creating a clear contrast between trabeculated and smooth atrial walls. A brief sequence shifts toward the left atrial appendage (auricula sinistra), where pectinate muscles appear more confined, with the posterior left atrial wall remaining comparatively smooth. Orientation matters here. The crista terminalis marks the internal ridge corresponding to the sulcus terminalis externally, and the animation makes its superior relationship to the sinoatrial node region easy to grasp while the pectinate bundles fan out toward the atrioventricular junction. It also sets up a common teaching pitfall: learners often expect the left atrium to look equally ridged, but most of its chamber derives from incorporation of pulmonary veins, so the pectinate pattern is largely limited to the auricle. Small structures, big consequences. In atrial fibrillation, the left atrial appendage is the dominant site of thrombus formation, and appreciating its trabeculated interior helps explain why transesophageal echocardiography and appendage occlusion strategies focus attention there.
