Description
Radiographic, semi-transparent rendering situates a pancreatic ductal adenocarcinoma within the upper abdomen of an adult male, with the pancreas shown retroperitoneal and transversely oriented, tucked posterior to the stomach and anterior to the aorta. As the sequence advances, the tumor enlarges and infiltrates along the pancreatic parenchyma toward the head region adjacent to the C-loop of the duodenum, bringing the common bile duct and main pancreatic duct into view. Vascular relationships are emphasized, with the lesion abutting or wrapping the superior mesenteric artery and vein, the portal venous confluence, and the splenic vessels coursing along the superior border toward the spleen. Pancreatic carcinoma is defined as much by what it invades as by where it starts. Progressive contact with the superior mesenteric vessels and portal vein explains why borderline resectable disease often hinges on degrees of arterial encasement or venous narrowing on CT and fluoroscopic-style projections, and why venous reconstruction may be planned while arterial involvement can preclude a standard pancreaticoduodenectomy. Ductal obstruction is another teaching point: a head-of-pancreas mass compressing the intrapancreatic common bile duct correlates with painless obstructive jaundice and upstream biliary dilatation, while posterior extension toward the celiac plexus helps explain refractory epigastric pain radiating to the back.
