Description
Posterior to the renal hila, the renal plexus appears as a yellow autonomic nerve network clustered around the origin and course of each renal artery as it branches laterally from the abdominal aorta. Fibers converge from the aorticorenal and adjacent prevertebral ganglia, then track distally with the renal artery toward the segmental branches within the renal sinus, maintaining a close perivascular relationship. The inferior vena cava and renal veins lie immediately anterior to much of this plexus, while the sympathetic trunks and lumbar splanchnic nerves sit more medial and posterior along the vertebral column. Small green lymphatic channels and nodal tissue can be expected near the renal vessels, paralleling the neurovascular pedicle. A posterior perspective matters because it aligns with how the renal autonomic supply is encountered in retroperitoneal dissection, where nerves often blend with adventitia and are easiest to appreciate when the kidney and great vessels are viewed from behind. Denervation is not theoretical here: catheter-based renal sympathetic denervation targets periarterial sympathetic fibers in the renal plexus to treat selected cases of resistant hypertension, and the proximity to the renal artery explains both the intended effect and potential vascular complications. Pain referral from renal colic also tracks these pathways, with sympathetic afferents traveling back through lumbar splanchnic nerves to spinal segments classically associated with flank and groin distribution.
