Description
Oriented from an inferior aspect, the heart is presented with the diaphragmatic surface of the ventricles facing the viewer and the coronary circulation mapped across the epicardium. The right coronary artery courses in the right atrioventricular groove toward the crux, where the posterior interventricular artery (posterior descending artery) typically descends inferiorly toward the apex, paired by the middle cardiac vein in the posterior interventricular sulcus. Along the left side, the circumflex branch of the left coronary artery tracks in the left atrioventricular groove, while tributaries of the coronary venous system converge toward the coronary sinus on the posterior aspect of the heart, running transversely and slightly superior to the posterior left atrium. Capillary-level surface branches are rendered as fine epicardial networks. Small vessels matter here. An inferior view clarifies territories that drive posterior and inferior wall ischemia, and it makes dominance patterns easier to teach: in right-dominant circulation the right coronary artery supplies the posterior interventricular artery, whereas left dominance shifts that supply to the circumflex. This is the surface anatomy that underlies ECG patterns in inferior STEMI (often RCA) and informs catheter engagement during coronary angiography, as well as surgical graft planning to the PDA or obtuse marginal branches when the posterior circulation is culprit. Cardiac anatomy and physiology courses use this angle to connect grooves, sulci, and vessel paths without the distraction of an anterior sternocostal emphasis, and it slots cleanly into interventional cardiology or cardiothoracic surgery materials discussing coronary dominance, the crux, and posterior revascularization targets. It also works well for patient-facing education on coronary artery disease when you need to explain why posterior symptoms can track with specific coronary segments.
