Description
Cut through the cranial base, the internal carotid artery is traced as it ascends from the petrous temporal bone within the carotid canal, turns anterosuperiorly through its cavernous segment, and then courses medially toward the supraclinoid region near the sella turcica. Bony landmarks of the skull base frame the vessel, including the petrous ridge, the body of the sphenoid, and the margins of the foramen lacerum region, with dentition and the lower facial skeleton preserved inferiorly to maintain orientation. Red arterial pathways spread across the endocranial surface, consistent with the proximal anterior and middle cerebral circulations, while blue channels mark adjacent venous spaces and sinus relationships. Following the carotid siphon in situ matters because this is where anatomy drives both symptoms and approach: the artery’s tight curvature within the cavernous sinus places it next to cranial nerves III, IV, V1, V2, and VI, so a cavernous carotid aneurysm or carotid-cavernous fistula can present with painful ophthalmoplegia, chemosis, and pulsatile proptosis. That same segment is also a key corridor in endoscopic endonasal and transsphenoidal surgery, where carotid localization relative to the sphenoid sinus and sellar floor sets the margin between safe exposure and catastrophic hemorrhage. Small distances here, big consequences. Neuroscience and head and neck anatomy courses can use this cross-section to teach the petrous, cavernous, and supraclinoid segments of the internal carotid artery alongside skull base landmarks that students must identify on CTA, MRA, and angiography.
