Description
Cut away from the calvaria, the inner table of the frontal, parietal, and occipital bones frames the cranial cavity and exposes the dural venous sinuses highlighted in blue. Along the midline, the superior sagittal sinus runs anteroposteriorly in the attached margin of the falx cerebri, ending posteriorly at the confluence of sinuses near the internal occipital protuberance. Circumferentially around the foramen magnum, the marginal sinus forms a venous ring on the inferior aspect of the posterior cranial fossa, positioned at the rim of the foramen and communicating with the occipital sinus and the internal vertebral venous plexus. Bony landmarks, including grooves for the transverse and sigmoid sinuses on the occipital and temporal bones, help orient the posterior drainage pathways. The marginal sinus matters when you want to teach or plan around the craniovertebral junction, where venous channels can be a quiet source of brisk bleeding. Midline suboccipital craniectomy, foramen magnum decompression for Chiari I malformation, and transcondylar or far-lateral approaches all traverse territory where the marginal sinus and occipital sinus may be enlarged or variant, and dural opening strategy changes accordingly. Bridging veins into the superior sagittal sinus also anchor discussions of subdural hematoma mechanics, but the ring-like marginal sinus is the surgical surprise. Expect variability.
