Description
Centered within the middle cranial fossa, the paired cavernous sinuses sit lateral to the body of the sphenoid and sella turcica, flanking the pituitary fossa on either side of the midline. From a superior viewpoint across the opened cranial cavity, the superior sagittal sinus tracks anteroposteriorly in the falx cerebri toward the confluence of sinuses, then continues bilaterally as the transverse sinuses along the attached margin of the tentorium cerebelli before curving inferiorly into the sigmoid sinuses en route to the jugular foramina and internal jugular veins. The cavernous sinus region lies anterior and medial to the petrous temporal bones, where venous channels in the dura converge around the parasellar compartment. Midline orientation is clear. Clinical teaching tends to focus on the cavernous sinus because it is one of the few dural venous sinuses that contains both venous blood flow and neurovascular structures in its wall and lumen, a configuration that explains the characteristic cranial neuropathies seen with cavernous sinus thrombosis. Lesions here can produce ophthalmoplegia from involvement of cranial nerves III, IV, and VI, along with facial sensory loss in the ophthalmic and maxillary divisions of the trigeminal nerve, and sympathetic disruption can yield a partial Horner syndrome. A superior view also helps you track venous drainage patterns, including how infection from the face can spread via the superior ophthalmic vein to the cavernous sinus, and how outflow continues through the petrosal sinuses toward the sigmoid sinus and internal jugular vein. Use this illustration to anchor lectures in gross anatomy and neuroanatomy on the dural venous sinuses, tentorium cerebelli attachments, and the parasellar corridor, or to support clinical teaching on cavernous sinus syndrome, dural arteriovenous fistulae, and skull base surgical approaches near the sella.
