Description
Centered on the petroclival region of the internal cranial base, the inferior petrosal sinus is traced along the petro-occipital fissure as it runs from the cavernous sinus toward the jugular foramen. Adjacent bony landmarks include the petrous part of the temporal bone laterally and the clivus of the occipital bone medially, with the middle and posterior cranial fossae forming the surrounding floor. Several cranial nerves course nearby, including the abducens nerve (CN VI) as it traverses Dorello’s canal, and the glossopharyngeal, vagus, and accessory nerves (CN IX to XI) approaching the jugular foramen. Tight spacing. Clear relationships. This corridor matters because the inferior petrosal sinus is the primary venous channel sampled during inferior petrosal sinus sampling (IPSS) for ACTH-dependent Cushing syndrome, where catheter placement hinges on recognizing its connection between the cavernous sinus and the internal jugular vein. The same anatomy anchors skull base surgery around the petroclival junction and jugular foramen, where venous bleeding and lower cranial nerve morbidity rise quickly when dissection drifts lateral to the sinus or deep to the dural folds of the posterior fossa.
