Description
Running lateral to the tibia, the fibula is traced from its expanded head and neck just inferior to the knee down to the distal lateral malleolus, which brackets the talus at the ankle mortise. Proximally, the fibular head sits posterolateral to the tibial condyle, while distally the fibula descends slightly posterior to the tibia before flaring at the malleolus. Portions of the distal femur and the knee articular region are included superiorly, and the foot skeleton extends inferiorly to the calcaneus, metatarsals, and phalanges in a plantar-facing presentation. A blue-highlighted band parallels the fibular shaft and continues toward the lateral ankle, consistent with the course of the fibularis (peroneus) longus and brevis tendons as they pass posterior to the lateral malleolus. Clinically, this lateral leg and ankle emphasis is where bony and soft-tissue injury patterns intersect. The fibula is the attachment site for the lateral collateral ligament at the fibular head and for multiple septa and muscle origins along the shaft, but the distal relationship to the talus is what matters when you are judging syndesmotic integrity, lateral malleolar fractures, or a Maisonneuve pattern with proximal fibular injury after an external rotation ankle sprain. Small shifts here change the mortise. Tenderness at the fibular neck also raises concern for common fibular nerve involvement, which can be missed when attention stays on the ankle.
