Description
Centered on the anterior knee joint, the patella (kneecap) sits displaced laterally relative to the femoral trochlear groove, with the medial patellar facet no longer congruent with the trochlear cartilage. Superiorly, the quadriceps tendon approaches the patellar base, while inferiorly the patellar ligament continues from the patellar apex toward the tibial tuberosity on the proximal tibia. The distal femur forms the medial and lateral condyles behind the extensor mechanism, and the menisci interpose between femoral condyles and tibial plateau, their crescent profiles framing the joint line. Articular cartilage caps the femur and tibia where contact and shear concentrate during dislocation. Lateral patellar dislocation in a male athlete most often follows a valgus load with the knee near extension, and the anteromedial soft tissues, commonly the medial patellofemoral ligament and medial retinaculum, are the usual sites of tearing. That injury pattern matters because a first-time dislocated patella can reduce spontaneously while leaving behind osteochondral defects on the lateral femoral condyle or medial patellar facet, a frequent cause of persistent effusion and mechanical symptoms. Alignment cues in this view help explain why a shallow trochlea, patella alta, or an increased Q-angle shifts the patella laterally during quadriceps contraction. Pain is expected.
