Description
Anterior surface anatomy is used to overlay the paired kidneys within the male abdomen, with the left kidney shown bearing a focal renal cell carcinoma centered in the parenchyma beneath the renal capsule. The renal pelvis lies medially at the hilum, continuous inferiorly with the ureter, which descends along the posterior abdominal wall toward the urinary bladder. Superior poles sit closer to the diaphragm, while lower poles project more inferiorly, a relationship that matters when comparing a pole-based mass to adjacent retroperitoneal structures. Renal cell cancer arises from renal tubular epithelium at the nephron level and expands as a solid cortical mass that can distort the collecting system rather than originate from it, helping learners separate RCC from urothelial tumors of the renal pelvis. Pole location guides operative strategy: a small upper or lower pole lesion may be amenable to partial nephrectomy with attention to segmental arterial branches, while a more central or hilar mass often pushes surgeons toward radical nephrectomy because of proximity to the renal vessels and collecting system. Hematuria, flank pain, and a palpable mass form the classic triad, but many cases present incidentally on CT, where recognizing cortical origin and capsular contour becomes the key teaching point.
