Description
Centered on the right retroperitoneum, the right kidney sits lateral to the abdominal aorta and posterior to the inferior vena cava, with the right suprarenal (adrenal) gland perched on its superior pole. Autonomic fibers of the renal plexus form a periarterial mesh around the right renal artery as it courses laterally from the aorta toward the renal hilum, with companion veins returning medially to the IVC. Inferiorly, the renal pelvis narrows into the ureter, which descends along the psoas region toward the pelvis. Lymphatic channels track with the renal vessels and converge toward paraaortic and paracaval nodes. An anterior perspective clarifies how the renal plexus follows the renal artery and its segmental branches rather than taking a straight route from the sympathetic chain. That relationship matters during nephrectomy and renal artery reconstruction, where periarterial dissection can interrupt sympathetic and visceral afferent fibers and contribute to postoperative flank pain or altered renal hemodynamics. It also helps explain referred pain patterns from ureteric obstruction, since visceral afferents from the kidney and proximal ureter travel back through this plexus to spinal segments that map to the loin-to-groin distribution.
