Description

Anteriorly, the right kidney is presented with its fibrous capsule and cortical surface, while a lacework of renal lymphatics tracks along the capsular margin and converges toward the hilum. Superior to the upper pole sits the suprarenal (adrenal) gland. Medially, the abdominal aorta and inferior vena cava flank the kidney, giving rise to the renal artery and renal vein, and the ureter descends inferiorly from the renal pelvis toward the pelvis. Lymphatic drainage of the kidney is easy to forget because it is rarely seen in routine imaging, yet it matters when disease escapes the parenchyma and travels along perivascular routes to the lateral aortic (lumbar) nodes. Tumor staging in renal cell carcinoma, postoperative lymphocele risk after nephrectomy, and atypical patterns of spread all hinge on the fact that renal lymphatics accompany the renal vessels at the hilum and communicate with perirenal and paraaortic channels. The anterior perspective also reinforces the relationship of these delicate vessels to the renal capsule, the adrenal region, and the great vessels. Small structures. Big consequences.
