Description
Posteriorly, the left kidney sits lateral to the abdominal aorta and inferior vena cava, capped superiorly by the suprarenal (adrenal) gland and enclosed by the renal capsule overlying the cortex. From the renal hilum, the renal pelvis funnels medially into the proximal ureter, while the renal artery and renal vein course transversely to meet the great vessels. Fine neural branches of the renal plexus track along the renal artery toward the renal sinus, with contributing fibers continuous with the celiac plexus superiorly and the aortic (intermesenteric) plexus along the anterior aspect of the aorta. A posterior view clarifies how autonomic fibers reach the kidney: sympathetic input travels via thoracic splanchnic pathways to the prevertebral ganglia and then redistributes through periarterial plexuses, while visceral afferents return along the same routes, a pattern that explains referred flank pain in acute ureteric obstruction and capsular distension. It also matters in the operating room, because dissection near the renal hilum during nephrectomy, renal artery aneurysm repair, or donor nephrectomy risks disrupting these periarterial nerve bundles, contributing to postoperative denervation effects and altered renal vascular tone. Small structures, big consequences. Use this plate for teaching retroperitoneal anatomy in gross anatomy and urology blocks, for illustrating autonomic supply of the kidney in physiology or pain referral discussions, and for surgical atlases covering posterior approaches and hilar control where the relationship of the renal plexus to the renal vessels must be communicated cleanly.
