Description
Positioned retroperitoneally on either side of the vertebral column, the paired kidneys are shown from a posterior perspective with the adrenal (suprarenal) glands capping their superior poles. Renal veins emerge from the renal hila and course medially to drain into the inferior vena cava, which lies to the right of the abdominal aorta, while the renal arteries arise laterally from the aorta and pass toward each hilum. Inferiorly, each ureter descends from the renal pelvis toward the pelvis, and renal capsule and outer cortex are represented to orient the vascular pedicle in relation to the parenchyma. Yellow autonomic nerve pathways track along the great vessels and into the renal hilum. Posterior anatomy makes the hilar relationships easier to teach because you can follow the renal vein’s short, thin-walled course to the inferior vena cava without the visual clutter of anterior abdominal viscera. Left sided venous anatomy matters. The left renal vein is classically longer, receives tributaries such as the left gonadal and left suprarenal veins, and it can be compressed between the superior mesenteric artery and aorta in nutcracker syndrome, a pattern that helps explain hematuria or left varicocele in men. Surgical exposure during nephrectomy or renal transplantation planning also hinges on the vein’s position relative to the artery and ureter at the hilum (anterior to posterior, vein then artery then pelvis).
