Description
Emerging from the pontomedullary junction, the abducens nerve (cranial nerve VI) runs anteriorly along the clivus in the prepontine cistern before angling superiorly to enter Dorello’s canal beneath the petrosphenoidal ligament. From there it passes through the cavernous sinus, positioned lateral to the internal carotid artery, and continues anteriorly into the orbit via the superior orbital fissure to supply the lateral rectus muscle. Close neighbors in this region include the oculomotor nerve (CN III) and trochlear nerve (CN IV) as they course toward the orbital apex, with CN IV typically more superior and lateral at the tentorial edge. Small nerve, long route. That anatomy matters because CN VI has the longest subarachnoid course of the ocular motor nerves, so raised intracranial pressure commonly produces a false localizing abducens palsy with impaired abduction and horizontal diplopia. The segment at the petrous apex is also exposed in skull base disease, classically in Gradenigo syndrome (petrous apicitis) where lateral rectus weakness can accompany deep facial pain from trigeminal involvement. Within the cavernous sinus, combined deficits of CN III, CN IV, CN V1, and CN VI help distinguish cavernous sinus thrombosis or carotid cavernous fistula from isolated orbital pathology. Use this illustration in neuroanatomy and head and neck teaching to anchor the CN VI pathway from brainstem to extraocular muscle, and in neurology or ophthalmology materials explaining patterns of ocular motility deficit and diplopia localization.
