Description
Arising from the ventral midbrain in the interpeduncular fossa, the oculomotor nerve (CN III) courses anteriorly and slightly laterally between the posterior cerebral artery superiorly and the superior cerebellar artery inferiorly, then runs forward toward the cavernous sinus. From an inferior perspective, the cerebral peduncles frame CN III medially, while the pons and basilar artery sit more posterior and inferior in the field. The optic chiasm and infundibular region lie anterior to the midbrain, and the cerebellum occupies the posterior fossa behind the brainstem. Color-coded cranial nerves radiate from the brainstem, helping orient CN III among its neighbors at the skull base. That tight arterial corridor is not a trivia point, it is the anatomy behind classic oculomotor palsy patterns. A posterior communicating artery aneurysm can compress the superficial parasympathetic fibers of CN III and produce an early, fixed dilated pupil with ptosis and a “down and out” eye, while microvascular ischemia more often spares pupillary function. Neurosurgeons and neurointerventionalists track this relationship during aneurysm evaluation at the circle of Willis and during approaches that expose the interpeduncular cistern, where a few millimeters decide whether CN III is mobilized safely.
