Description
Arising from the ventral midbrain within the interpeduncular fossa, the oculomotor nerve (cranial nerve III) emerges just medial to the cerebral peduncles and courses anteriorly toward the cavernous sinus on its way to the orbit. From a superior posterior perspective, the paired CN III roots can be appreciated in relation to neighboring brainstem landmarks, with the midbrain superior to the pons and medulla oblongata and the cranial nerve rootlets radiating bilaterally from the brainstem surface. Other cranial nerves are likely visible as symmetrical yellow projections along the pontomedullary junction and lateral medulla, creating a positional map of the cranial nerve exit zones. Small structure, big consequences. That interpeduncular emergence point is a key localization clue in neuroanatomy and bedside neurology, because compressive lesions near the tentorial edge or posterior communicating artery can impinge CN III early in its cisternal course. Pupillary involvement helps. A dilated, poorly reactive pupil with ptosis and an eye resting “down and out” steers you toward a compressive third nerve palsy rather than a microvascular ischemic palsy, and this view supports teaching the nerve’s proximity to the midbrain and adjacent cranial nerve exits. Neuroanatomy courses, ophthalmology and neurology board review materials, and neurosurgical education pieces on aneurysm-related cranial neuropathies will all benefit from this clean cranial nerve topography.
