A distinct subarachnoid space separates arachnoid from pia mater on the cortical surface, and it is traversed by arachnoid trabeculae and crossed by superficial cerebral arteries and veins coursing over the gyri. Superiorly the arachnoid is apposed to dura near the sagittal region, and inferiorly it follows the contour toward the temporal fossa, with cranial nerves emerging from the brainstem region toward the skull base. This lateral section is the cleanest way to teach why cerebrospinal fluid resides in the subarachnoid space and not between dura and arachnoid, a point that matters when differentiating subdural hematoma from subarachnoid hemorrhage on CT and MRI. Blood within the basal cisterns and along cortical sulci tracks the same CSF pathways illustrated here, and vasospasm after aneurysmal rupture is easier to explain when the relationship of vessels to the arachnoid and pia is explicit.
