Description
Running along the medial malleolus, the great saphenous vein ascends anterior to the Achilles tendon and continues proximally along the medial border of the tibia, receiving tributaries from the medial dorsal venous network and the medial marginal vein of the foot. Distally, the medial longitudinal arch is framed by the calcaneus, talus, navicular, and the cuneiforms with the first metatarsal and phalanges extending anteriorly. Plantar to these bones, tendons of tibialis posterior, flexor digitorum longus, and flexor hallucis longus course toward the midfoot and forefoot, while the calcaneal (Achilles) tendon inserts on the posterior calcaneal tuberosity. Clear relationships. A medial vascular view matters because it aligns with the sites clinicians palpate, cannulate, and decompress around the ankle and tarsus. The great saphenous vein is a common conduit for bypass grafting and a frequent target for venous access, but its proximity to the saphenous nerve along the medial leg and ankle explains why vein harvesting or phlebectomy can leave a strip of medial foot numbness. In chronic venous insufficiency, reflux in the great saphenous system often correlates with edema and skin change around the medial malleolus, and this illustration helps link surface findings to the underlying venous pathway.
