Description
Running from the distal tibia and fibula into the pes, the anterior tibial artery continues as the dorsalis pedis on the dorsum while the posterior tibial artery passes posterior to the medial malleolus to divide into the medial and lateral plantar arteries on the sole. Distally, dorsal and plantar metatarsal arteries course between the metatarsals toward the digital arteries of the toes, with accompanying veins draining into the dorsal venous arch and then proximally toward the great and small saphenous pathways. Deep to the vascular network, the tarsus, metatarsals, and phalanges provide the rigid framework, while extensor and flexor tendons span the ankle to insert on the phalanges and plantar ligaments reinforce the longitudinal arch. This combined osteomuscular and vascular rendering matters because pedal circulation is assessed and treated along named segments, and the dorsalis pedis pulse (lateral to the extensor hallucis longus tendon) remains a frontline landmark in peripheral arterial disease.
