Description

Centered in a frontal, radiograph-style presentation, the trachea descends in the midline from the laryngeal inlet and bifurcates at the carina into the right and left main bronchi. The right main bronchus courses more vertical and wider toward the right hilum, then divides into lobar (secondary) bronchi for the superior, middle, and inferior lobes, while the left main bronchus angles more obliquely to supply superior and inferior lobar bronchi. Segmental (tertiary) bronchi radiate distally within each lobe, branching centrifugally toward the peripheral lung, with the airway tree shown in high contrast against semi-transparent lung fields. A clear airway map. Bronchial anatomy is not symmetric, and that asymmetry drives clinical decision-making. The more vertical right main bronchus explains why aspirated material and malpositioned endotracheal tubes more often enter the right lung, producing right upper lobe or dependent right lower lobe atelectasis on imaging. Distal branching patterns also underpin bronchoscopy navigation and segment-based interventions, including endobronchial biopsies, bronchial blocker placement, and targeted segmentectomy planning, where accurate identification of lobar and segmental bronchi prevents wrong-segment sampling.
