Description
Beneath the superficial fascia of the male cheek, the buccinator occupies the muscular plane of the lateral wall of the oral cavity, extending from the maxillary and mandibular alveolar processes posteriorly toward the modiolus at the angle of the mouth. Its fibers lie deep to the skin, subcutaneous fat, and the risorius and zygomaticus major, while remaining anterior to the masseter, with the parotid duct coursing across the masseter before turning medially to pierce the buccinator opposite the maxillary second molar. Laterally, the buccinator relates to the buccal fat pad and facial vessels; medially it is lined by buccal mucosa, shaping the vestibule between cheek and teeth. Cheek muscle anatomy is easy to oversimplify, but the buccinator is the structure that turns the cheek into an active wall rather than passive soft tissue. Buccinator weakness after facial nerve (buccal branch) injury leads to food trapping in the vestibule, and the muscle also defines the safe and unsafe depth for intraoral incisions, where you want to stay superficial to it when accessing the buccal space. Surgeons also orient to the point where the parotid duct penetrates the buccinator, a fixed landmark that helps avoid duct injury during laceration repair or transbuccal approaches.
