Description
Seen from a lateral head and neck perspective beneath the cheek skin, the buccinator forms a broad, thin muscular sheet spanning from the maxillary and mandibular alveolar processes and the pterygomandibular raphe toward the modiolus at the oral commissure. Anteriorly it interdigitates with orbicularis oris, while posteriorly it lies deep to the masseter and is separated from it by the buccal fat pad and the parotid (Stensen) duct as that duct turns medially to pierce the muscle. Superiorly the fibers relate to the zygomaticus group at the corner of the mouth, and inferiorly they blend with depressor anguli oris along the perioral musculature. A cheek muscle, not a chewing muscle. Functionally, buccinator matters because it positions the bolus on the occlusal surfaces by pressing the cheek against the teeth, so weakness leads to food trapping in the vestibule and a characteristic “pocketing” complaint after facial nerve (CN VII) injury. The lateral view also clarifies a common procedural hazard: the parotid duct’s relationship to buccinator and the molar region, a practical landmark for avoiding duct injury during cheek laceration repair, buccal mucosa surgery, or dental anesthesia in the maxillary molar region.
