Description
Sectioned male calvaria in lateral profile exposes the posterior cranial fossa dura and the torcular Herophili (confluence of sinuses) at the internal occipital protuberance. From this venous crossroads, the superior sagittal sinus courses superiorly in the midline toward the vertex, the straight sinus approaches from anterosuperior along the junction of falx cerebri and tentorium cerebelli, and paired transverse sinuses sweep laterally along the attached margin of the tentorium toward the sigmoid sinuses. Superficial cerebral veins and cortical arterial branches spread over the convexity, with the cerebral hemispheres occupying the anterior and superior aspects of the exposed cranial cavity. Torcula anatomy matters because it is where changes in intracranial venous outflow can present first, and small variations in dominance between right and left transverse sinuses influence both symptom patterns and procedural planning. Venous sinus thrombosis often involves the superior sagittal sinus or transverse sinus, but propagation through the confluence explains why headache, papilledema, and hemorrhagic venous infarcts can coexist. For posterior fossa and occipital craniotomies, the relationship of the confluence to the internal occipital protuberance and tentorial attachment is a surgical map, and injury here can cause rapid blood loss and difficult-to-control air embolism. A hard place to tamponade.
