Description
Running superficially along the posterior calf, the small (lesser) saphenous vein begins behind the lateral malleolus and ascends in the midline toward the popliteal fossa, closely related to the calcaneal (Achilles) tendon inferiorly and the gastrocnemius and soleus muscles deep to it. Lateral and medial tributaries on the posterior leg converge on its trunk, while the great saphenous vein is also visible on the medial side, allowing comparison of the two principal superficial venous channels. Deep to the superficial fascia, portions of the tibia and fibula contour the leg, and the calcaneus and ankle region appear with the feet held in mild plantar flexion. Spatial relationships are clear. Clinically, this posterior view matters because the small saphenous vein commonly terminates at the saphenopopliteal junction, a site with frequent anatomic variation that influences duplex ultrasound mapping and operative planning for varicose vein treatment. Reflux in the small saphenous system produces posterior calf varicosities and can be missed if attention stays on the great saphenous territory. The close relationship to the sural nerve along the posterior-lateral calf also frames a real procedural hazard during endovenous ablation, phlebectomy, or surgical ligation. Nerve symptoms happen.
