Description

Running superficially along the medial aspect of the male lower limb, the great saphenous vein ascends from the dorsal venous arch of the foot, passes anterior to the medial malleolus, and continues up the medial leg and thigh toward the femoral triangle. Proximally, it pierces the cribriform fascia at the saphenous opening to join the femoral vein at the saphenofemoral junction, where superficial tributaries commonly converge (superficial epigastric, superficial circumflex iliac, and superficial external pudendal veins). A parallel superficial network is suggested in the integument, and regional channels and nodes in the superficial inguinal region may also be indicated, reflecting the close anatomic relationship between superficial venous and lymphatic pathways. Clinically, this is the workhorse superficial vein in chronic venous disease. Incompetence at the saphenofemoral junction and along the saphenous trunk drives the classic pattern of medial thigh and calf varicosities, and this course also explains why duplex ultrasound mapping tracks the vein from the medial malleolus to the groin when planning endovenous thermal ablation or foam sclerotherapy.
