Description
Running subcutaneously along the medial foot, leg, and thigh, the great saphenous vein is traced from the dorsal venous arch anterior to the medial malleolus, then up the medial border of the tibia toward the knee and proximal thigh. Superiorly, it approaches the femoral triangle to join the femoral vein at the saphenofemoral junction, a short distance inferior to the inguinal ligament. Deeper in the thigh, the femoral vein parallels the course of the superficial channel, while the small saphenous vein is suggested more posterolaterally and the popliteal vein lies posterior to the knee joint line. An anterior perspective on the great saphenous vein matters because it fixes the surface anatomy used for venous access and for correlating duplex ultrasound findings with palpable landmarks. Varicose vein disease often reflects valvular incompetence at the saphenofemoral junction or along medial thigh tributaries, and this course also explains the typical distribution of venous ulceration and lipodermatosclerosis around the medial distal leg.
