Description
Running in the superficial fascia along the medial side of the limb, the great saphenous vein is traced from the dorsal venous arch of the foot, anterior to the medial malleolus, then ascending along the medial tibia, crossing the knee region, and continuing up the medial thigh to the saphenofemoral junction near the femoral triangle. A lateral perspective still allows the long subcutaneous course to read clearly against deeper landmarks, including the tibia and fibula, the patella and distal femur at the knee, and the tarsals and metatarsals distally. Red arterial pathways course deeper and more centrally, contrasting with the blue superficial venous channel. Perforating veins and small venules can be inferred where superficial and deep systems communicate. Clinically, this is the vein you map for varicose disease and for planning endovenous ablation, where reflux at the saphenofemoral junction or along thigh tributaries drives tortuosity and symptoms. Its relationship to the medial malleolus and medial tibial border also matters during cutdowns, trauma, and bypass conduit harvest, and it frames a key teaching point: superficial veins lie in subcutaneous tissue while the deep venous system accompanies arteries as venae comitantes. Lateral viewing helps learners appreciate why the vessel remains relatively protected from direct tibial impacts yet vulnerable at the ankle and knee where superficial soft tissue coverage thins.
