Description
Running superficially along the medial side of the lower limb, the great saphenous vein is traced from the medial marginal vein of the foot, anterior to the medial malleolus, then proximally along the medial tibia and knee to the medial thigh. From a lateral perspective, its subcutaneous course is appreciated as it passes anterior to the medial femoral condyle and continues toward the saphenous opening in the proximal thigh. Short tributaries and communicating venules converge along its length, suggesting connections with the posterior arch vein and perforating veins that link superficial to deep compartments. Orientation is anchored by the groin and medial ankle landmarks. This side-on view matters because the great saphenous vein is the common substrate for chronic venous insufficiency and varicosities, with reflux often originating near the saphenofemoral junction and propagating distally along the medial thigh and leg. Surgeons and interventionalists planning endovenous thermal ablation, foam sclerotherapy, or high ligation need a clear mental map of where the trunk lies relative to the knee and ankle landmarks, and where tributaries and perforators may complicate access or recurrence. Preoperative vein harvest for coronary artery bypass grafting also depends on respecting its superficial course while avoiding injury to the adjacent saphenous nerve along the medial leg.
