Description
Running from the medial marginal vein of the foot, the great saphenous vein ascends anterior to the medial malleolus, then tracks proximally along the medial border of the tibia and knee before continuing up the medial thigh to the saphenofemoral junction in the groin. In a true lateral profile of the male lower limb, its course is read against the contour of the calf and thigh, with the vein remaining superficial to the deep fascia while the femoral and popliteal veins lie deeper and more central. Perforating veins are implied where superficial tributaries angle posteriorly and anteriorly to communicate with the deep venous system. Key surface landmarks anchor the pathway. Clinically, this is the workhorse vein for both pathology and access. Primary varicose veins often reflect valvular incompetence at the saphenofemoral junction or at incompetent perforators around the medial calf, and a lateral view helps clarify how reflux can still present with visible tortuosity despite the vein’s fundamentally medial track. Surgeons planning endovenous thermal ablation or high ligation use this relationship to the medial malleolus, medial femoral condyle, and groin crease for safe cannulation and to anticipate where the saphenous nerve runs in close company with the vein along the medial leg. Nerve irritation is a known complication.
