Description

Shown in internal cross section, the male kidney is opened to expose the renal cortex peripherally and the medulla with its renal pyramids converging toward the papillae. Minor and major calyces coalesce into the renal pelvis, which narrows inferiorly into the ureter at the hilum. Fine renal lymphatic vessels course with the segmental branches and then converge medially alongside the renal artery and renal vein within the renal sinus. Their alignment with the hilar structures is the point. Renal lymphatics matter because they do not wander far from the vessels, they drain toward paraaortic (lumbar) lymph nodes and can provide a pathway for tumor spread or postoperative lymphatic leak. In radical nephrectomy and nephroureterectomy, the lymphatic channels clustered at the hilum and around the renal vessels are the ones encountered during vascular control and nodal dissection, and they help explain why paraaortic nodal disease can accompany renal cell carcinoma even when the kidney parenchyma appears confined. For teaching, the sectional anatomy also reinforces how the collecting system sits central and medial, while the lymphatics follow the vasculature rather than the calyces.
