Sectioned along an anterior plane in an adult male, the kidney is opened to expose the renal cortex peripherally and the medulla centrally, with renal pyramids tapering toward the papillae and minor calyces. Fine renal lymphatic vessels course with the interlobar and arcuate arteries and veins, then converge toward the renal sinus where larger collecting trunks track to the renal hilum. Lymph nodes are shown adjacent to the hilar vessels, positioned medial to the renal parenchyma and proximal to the origin of the ureter at the renal pelvis. Small but telling anatomy. Attention to the renal lymphatics matters because lymphatic drainage follows the renal vasculature and provides a pathway for spread of renal cell carcinoma toward the paraaortic and paracaval nodal basins, even when the primary mass remains confined within the cortex.
