Description

Sectioned in a sagittal plane, the male kidney is opened to expose the renal cortex and medulla with medullary pyramids converging toward the renal papillae, minor calyces, and renal pelvis. Fine renal lymphatic vessels form a plexus within the parenchyma, then track centrally alongside interlobar and arcuate vessels toward the renal sinus and hilum, where they accompany the renal artery and renal vein. Anterior orientation is maintained so the hilar structures read in their expected relationships, with the collecting system positioned medial to the parenchyma and the hilar conduit directed toward the proximal ureter. Small nodal elements are implied along the hilar outflow. Renal lymphatics matter because they define the principal routes for tumor and infection spread from the kidney, and they help explain why renal cell carcinoma often reaches paraaortic and paracaval lymph nodes even when the primary mass appears confined on imaging. Lymphatic channels closely follow the renal vasculature, so lymphadenopathy can cluster around the renal pedicle and obscure surgical planes during nephrectomy or partial nephrectomy. A clean sagittal cross section also clarifies how parenchymal drainage converges at the renal sinus, a point that gets lost in external surface views. This is where staging starts.
