Description
Positioned on the medial aspect of the female breast, the highlighted arterial channel courses from the parasternal region into the mammary gland, sending smaller arterioles between lobules and along the lactiferous ducts toward the nipple and areola. Yellow glandular lobules sit embedded within adipose tissue, tethered to the dermis by Cooper’s ligaments that run anterior to the pectoralis major fascia. Posteriorly, the breast rests on pectoralis major, with pectoralis minor deeper and more superior, and a venous network accompanies the arterial supply in parallel. Medial breast perfusion matters because it commonly derives from perforating branches of the internal thoracic artery, a key source during oncoplastic resections, reduction mammoplasty, and perforator-based reconstruction planning. Injury to these medial perforators can compromise skin and gland viability or complicate hemostasis, while their relationship to the parasternal lymphatic drainage corridor intersects with patterns of tumor spread and sentinel node mapping. A clean medial-to-lateral vascular read is a teaching win. It also anticipates why parasternal approaches demand respect for intercostal spaces and perforator caliber.
